In order to register your child you will need to provide all of the following documents of each student:

• Birth Certificate
• Immunization Record (up to date)
• Proof of Residence (any of those: Apartment Lease, Light Bill or Water Bill)
• Picture ID of parents
• Proof of income for Pre-K program (any of those: Two most current check stubs. If you receive Food Stamp letter that show the amount you get. Or If you get paid cash a letter from your employer stating the amount you get paid signed by employer and dated).

Also complete the documents that are attached below.
