To make an appointment, you may use the Next Level app on your cell phone, call 281-869-3630, or make an appointment online. 

Follow these instructions to make an appointment online. 

• Go to Next Level Urgent Care
• Click the link where it says “Onsite Member? Click here.” 
• Select “Onsite Clinic” from the drop-down menu 
• Click on “Onsite Member” 
• On the Next Level Client Login page, log in using the following: 
  Username: HISD 
  Password: Nextlevel 

You will be brought to a page where you can select either the HMW clinic or the Attucks MS clinic. After you select the clinic, you have two choices: “Request Same-Day Sick Visit” or make an appointment with either Dr. Wan or another clinic staff member. If you request a same-day sick visit, fill out your name and additional information required, and someone at the clinic will call you to discuss your situation. Be sure to use your name as it appears on your HISD ID card and your correct date of birth so that all your clinic records will be in one place.
