• 7th and 8th Grade Parents:
    All 7th and 8th grade students must show proof of a Meningitis Vaccine (MCV4) as well as a Tdap Booster. Please make sure that your child receives these immunizations before the start of the school and provide us with a copy of the update record either during the summer or at the very start of the next school year (2017-2018).
    Students new to our school having a chronic health condition as well as those who never turned in documentation of a chronic health condition such as asthma, sickle cell, diabetes, life threatening food allergy etc. will need to have written documentation of the illness from their physician on file. Those students using inhalers, epi-pens or taking any medication at school will also need a new medication form completed by their physician and on file in the clinic before any medication can be given. Medication forms are available on the HISD health and medical services website or from the school nurse beginning August 20th.
