Dear Lanier Family,
I would like to extend a warm welcome to all students and their parents. I am looking forward to working with you and your children at Lanier Middle School during the 2018-2019 school year. To ensure your child’s health and safety I would like to share a few items with you.
MEDICATIONS: NO student is to take or carry prescription or over the counter medications to school. All medications must have a MEDICATION ADMINISTRATION form, must be completed and signed by both physician and the parent. This form along with the prescribed medication in its original container must be brought to school by the parent/guardian. Forms can be printed from https://www.houstonisd.org/Page/128845.
SCREENINGS: As per Texas State Law the following screenings will be done at Lanier Middle School:
VISION, HEARING & ANTES SCREENING: Will be conducted by the school nurse throughout the fall semester. You will be notified only if your child does not pass the screening.
SCOLIOSIS SCREENING: IMPOTANT! The Texas Department of State Health Services recently revised spinal screening rules to screen students by grade beginning in the 2018-2019 school year.
I am now required to screen 7th grade girls and 8th grade boys, this may require students being screened two years in a row.
FLU SHOTS: Information was sent home with your children about the upcoming Healthy Schools Flu Clinic to be held at Lanier on Tuesday, November 13, 2018 from 12:15 pm to 3:50 pm. Flu shots are free for all students. Return the consent form by October 5th to reserve your student’s space. Online registration: www.register.HISDflu.com
