Summary of Requirements & Instructions for Forms, Schedules & Statements

Please select the option below that is applicable to your bid/proposal and submit ONLY the forms pertaining to the option selected.

Option A   Select this option if you are a certified M/WBE.

Option B   Select this option if you will use M/WBE’s to assist in meeting HISD’s current M/WBE participation goal. (You will meet or exceed HISD’s participation goal level.)

Option C   Select this option if you will use M/WBE’s, but were unable to meet HISD’s participation goal. (You will not meet or exceed HISD’s participation goal level.) Good Faith Efforts documentation must be included with this option.

Suppliers that will be bidding/proposing as a Joint Venture, where at least one of the partners is an M/WBE should contact the Business Assistance Supplier Diversity Program  for specific instructions.

IMPORTANT - PLEASE NOTE THE FOLLOWING:

Submit ONLY the required M/WBE forms that pertain to the option you have selected. If applicable, attach supporting documentation. Failure to submit these forms and supporting documentation may result in the bid/proposal being considered "non-responsive" : An official of the supplier company must manually sign each form.

M/WBE firms must be certified as a minority- or women-owned enterprise by either the City of Houston, the Houston Minority Business Council (National Minority Business Council), or the Women's Business Enterprise Alliance - Houston (Women's Business Enterprise National Council).

If the total amount is less than $50,000 for Procurement and Professional Services, the supplier is not required to submit M/WBE forms, but is encouraged to meet HISD's M/WBE participation goal level.

If the total amount is less than $50,000 in Construction/Construction related projects, the supplier is not required to submit M/WBE forms, but is encouraged to meet HISD's M/WBE participation goal level.

For assistance in completing the M/WBE forms you may attend a workshop on the first Wednesday of each month at the CFS/Bond Administation Building located at 3200 Center Street, Houston, TX 77007. If the first Wednesday should fall on a scheduled holiday, the workshop will be moved to the next Wednesday that month. Contact our office for a list of scheduled dates and topics for these events.

If you wish to have your completed M/WBE participation forms reviewed prior to submitting your bid/proposal package, contact the Business Assistance Supplier Diversity Program at (713) 556-7273, or fax the forms to (713) 556-7274. Allow a reasonable amount of time for this review.

The Business Assistance Supplier Diversity Program's policy regarding these "Specific Conditions for M/WBE Participation" can be accessed electronically by clicking on the link in our "Forms and Documents" section or by obtaining a copy at the address cited in the "Specific Conditions for M/WBE Participation."

Once the award has been made, the supplier will be required to report M/WBE participation to the Office of Business Assistance on a periodic basis. Specific procedures and forms will be provided by the Business Assistance Supplier Diversity Program.
