- Child's original birth certificate (ORIGINAL BIRTH CERTIFICATE FOR PK)
- Child's Social Security Card
- Child's Immunizations (updated)
- Child's final report cart for the 2019-2020 school year (Grades Kinder - 5th grade)

(Current pay stub or a letter from employer stating gross wages paid and how often they are paid) in addition to the above items. Your child MUST be 3 years of age on or before September 1, 2021.
