     If you are zoned to Hartsfield Elementary, here is what you'll need to enroll your child: 
     
    If the scholar is in PK you will need the following to apply through our Head Start Office:

     Original Birth Certificate

     Shot Record – must be up to date

     Proof of Address – we only accept a gas, light, water bill, rent receipt or lease. If the bill is not in your name we will need a written statement from the person whose name is on the bill and a statement of residence.  NO phone, cell phone, cable or insurance bill

     Valid Texas Driver’s License or Identification Card

     Proof of Income- most recent check stub, child support, social security, income tax, TANAF or public assistance.

     

     Social Security Card – if your child has one

     

    You will need the following if student if the scholar is new to KN or 1st Grade:

     Last Report Card and/or Summer School Information

     Any information regarding special programs or special education/resource classes

     

     If the scholar attended a school outside of HISD, we will need the following:

     Original Birth Certificate

     Shot Record - must be up to date

     Proof of Address- gas, light or water bill, rent receipt, lease of home/apartment

     Last Report Card and/or Summer School Information

     Any information regarding special programs or special education/resource classes

     

    ALL ENROLLMENT REQUIREMENTS MUST BE COMPLETE.

     PARTIAL INFORMATION WILL NOT HOLD A SPOT FOR THE STUDENT.
