1.  A signed order from a doctor (see form below) is required for the school to administer any medication (even over the counter).

2.  Prescription medication must be brought in the original prescription bottle with the prescription label attached.

3.  Over the counter medication must be brought in an unopened container.

4.  Students are not permitted to bring their own medication to take on their own. All medication at school must be given by school staff and must have an accompanying order.

5.  Medications brought in will be kept at school. We are unable to send medication back and forth from home to school.

6.  Medication must be brought in by adults. Students are not allowed to have medication in their backpacks.

7. New medication order forms are required for each new school year.

8.  Medications that are taken three times a day or every 8 hours should be given at home, unless there are extenuating circumstances.

9.  Morning medication should be taken at home, unless there are extenuating circumstances.

Completed medication forms can be dropped off at school, emailed to LHALL9@houstonisd.org or faxed to 346-330-2528
