The following documents will be required to properly enroll all students:

• Social Security card (optional) or number
• Birth Certificate/Baptismal Certificate
• Immunization Record
• Verifiable apartment lease with student as an occupant or Deed of Trust with utility bills
• Report card/Test scores (current) from last school attended
• Parent or legal guardian proof of identification
