Medical conditions, temporary or ongoing, can affect a student’s ability to succeed at school. Some medical conditions require medications, procedures, treatments and/or special diet during the school day. For medications, procedures, treatments and diets during the school day there are Houston Independent School District forms that must be completed by the student’s doctor annually and cosigned by the parent or guardian. Furthermore, if the medical condition is ongoing and affects student’s performance in class or access to class then accommodations might be needed.
