EXTENDED DAY PROGRAM UPDATE

BEGINNING OF SCHOOL REQUIRED FORMS

Instead of a packet of paper forms, the beginning of the year forms are all digital this year.  Most forms can be accessed through Parent Connect.  If you have not yet created your account, go to hisdconnect.houstonisd.org/public and enter the access codes provided in the letter you received.  If you do not have your parent access code, please email Ms. Sims, ssims1@houstonisd.org.

There are also two required forms on our website, the Health Inventory and Food Allergy forms.  We need these forms for EVERY student.  You may complete them digitally and email them to Nurse Suiki or print them and drop them off in the school office.  They are listed in the Important Documents section of this page.
