The Americans with Disabilities Act of 1990 requires employers to provide “reasonable accommodations” to employees with a disability, physical or mental impairment, that limits one or more major life activities.

Please note, when submitting your request, you will need to submit a signed physician's statement, on official letterhead, that clearly provides the following information:
- Your medical diagnosis/condition (severity and nature),
- The activities that are impacted or limited by your diagnosis/condition,
- The recommended accommodations, and
- How long the accommodations are needed.
