If you are in need of a diploma replacement, simply download the form to your computer, fill out the form and press the submit button at the bottom of the form.  It will submit it to the Registrar via email.We will then begin the process of requesting a duplicate.  Once the duplicate diploma comes in, we will contact you.

• Replacement Diploma Fee is $25.00 due on Date of Order
• Replacement Diploma Cover Fee is $10.00 due on Date of Order
• Replacement Diplomas take 6 – 8 weeks after order date to arrive
• You will be contacted by telephone once Replacement Diploma is ready for pickup
• A photo ID is required to pick up Replacement Diplomas
