Enrollment Forms

    I hope everyone is getting adjusted after your first week of school.  I need your help with a couple of additional tasks to help make this a smooth-running year.  First, I need you to log on the parent portal, https://hisdconnect.houstonisd.org/public/, and make sure that information like your address, contact phone numbers, and parents’ emails are in the system.

    Second, parents must fill out the forms for the start of the school year.  We need you to complete the Family Survey, Home Language Survey, Student Contacts, Student Assistance Questionnaire, Military Connected Family Survey, Social Economic Information, and Student Change of Address form.  Go to the parent portal, https://hisdconnect.houstonisd.org/public/, find the tab on the left side that says Forms, click on it, and there are eight different documents that you need to click on to complete.  These forms are very important to our funding and helping your student with services that will guide them through a successful education.  These funds will help with provide tutorial service for students, free and reduced lunches, and even scholarship offers for graduating seniors.
