Can I cover my children?
Posted by:If you’re covered, your dependent children are automatically covered for 50% of the primary insured amount at no additional cost. However, dependent children are not eligible for the annual health-screening benefit.

Are there any limitations to the critical illness coverage?
Posted by:The Critical Illness benefit is available on a one-time basis, which means that if 100% of your selected benefit amount is paid, no further benefit is payable for the same or for any other covered critical illness.

Does critical illness insurance pay in addition to other insurance such as medical coverage?
Posted by:Yes. Critical illness benefits are in addition to benefits you receive from any medical plan and do not affect your medical benefits. This plan does not decrease benefit payments by coordinating with other plans to pay up to a certain maximum.

I already have cancer and specified diseases insurance. Why would I want critical illness coverage?
Posted by:The critical illness plan provides a lump sum benefit amount for stroke, heart attack, coronary artery bypass, organ transplant or end-stage renal failure in addition to some cancer coverage. The cancer and specified diseases plan provides for reimbursement of covered expenses for cancer treatments.

How do I enroll?
Posted by:You can enroll during annual benefits enrollment by visiting the HISD Benefits website. If you have a family status change, please call the HISD Benefits Service Center at 877-780-HISD (4473), choose a language and press 0. They are open weekdays (except holidays), 7 a.m. to 7 p.m. and Saturdays, 7 a.m to 4 p.m CST. More information about family status changes is available here.
Your new coverage is effective the first of the month following approval by the insurance carrier if you’re actively at work. If you sign up during annual benefits enrollment, your coverage can’t start before the new plan year begins.

How do I file a claim?
Posted by:To receive reimbursement for covered expenses, you must fill out the Critical Illness claim form, available on the HISD Benefits website Forms page. You can also call Aflac Group at 800-433-3036, 7 a.m. to 4 p.m. Central Time, weekdays (excluding holidays) and speak to a customer service specialist.
