• Registration for the 2014-2015 School Year
    Enrollment Requirements for Bellaire High School
     
     
    In order to enroll at Bellaire High School, a student MUST:
     
    Be zoned to Bellaire High School OR must have an approved transfer to Bellaire High School
    You must have ALL of the following records and documents to be enrolled. NO EXCEPTIONS.
     
    I. Parent or Legal Guardian (as appointed by the courts) must accompany the student.
        Legal documents must be provided in situations involving divorce and/or legal guardianship.
     
    II. Proof of address or parent or court appointed guardian
         1. If  living in a house, electric, gas, or water bill which shows both the service address and the billing address.
         2. If living in an apartment, a lease naming the parent or court appointed guardian and student as residents or a letter from the apartment manager on apartment letterhead or stationary. This letter should state who the student lives with and whatapartment. The letter must be signed by the apartment manager and contain a phone number where we may contact them.
     
    III. Proof of age/identity
         1. Birth certificate or passport
         2. Social Security card if the student has an assigned number
     
    IV. Health Records–Students CANNOT enroll if their immunization records are not up to date
         1.Diphtheria-Tetanus (DT)-Three (3) doses Booster within the last 10 years
         2. Polio Booster-Three doses, Last dose since the age of 4
         3. Measles, Mumps, Rubella (MMR)-Two doses; Both on or after the first birthday.
         4. Hepatitis B Series-Three doses
         5. Varicella (Chicken Pox)- Reliable history or 1 dose. 2 doses required if child 13 or older when the first dose was received.
     
     
     
    V. School Records from your last school
         1. Final report card with promotion status from this past school year is required of incoming 9th graders. Students earning high school credit in middle
             school must provide transcript with courses listed with credit awarded/earned.
         2. If the student will be a 10th-12th grader, a copy of an unofficial transcript is necessary.
         3. If the unofficial transcript is not available at registration, the student will be placed into 9th grade until the transcript can be produced.
         4. If registering during the school year, withdrawal papers and withdrawal grades from previous school are required.
         5. Students from other countries must provide school records from each year of secondary school beginning with year 7. These records must be  
             translated and include grading scale.
     
    During peak registration times allow a minimum of two hours. It is a slow process to check all of the paperwork.
