Financial support during difficult times
Supplementing your coverage
New and newly eligible employees may elect up to five times your annual base salary, to a maximum of $600,000, with no evidence of insurability (EOI) required. You may increase the amount of coverage by one times your base salary each year after that, also without EOI, until you reach five times your annual base salary (or $600,000, whichever is less).
Want even more coverage?
You may elect more coverage, up to eight times your annual base salary, to a maximum of $1,000,000; however, any amount over five times your salary (or $600,000, whichever is less) is subject to EOI regardless of when you make this election.
EOI is required for employees who don’t enroll when they’re first eligible, regardless of the multiple of salary elected.
Spouse and child coverage
If you enroll in supplemental life with a matching AD&D benefit amount for yourself, you can also purchase supplemental life with a matching AD&D benefit amount for your spouse and dependent child(ren).
Spouse coverage is available at one to three times your salary, equal to your coverage amount or $250,000, whichever is less. Note that any coverage above $100,000 is subject to EOI.
Child life and AD&D options are available at $5,000, $10,000, $15,000 or $20,000. A child may not be covered by more than one employee. Your cost for child life coverage is the same, no matter how many eligible children you insure.
Coverage options and cost
Coverage amounts are multiplied and then rounded to the next higher $1,000. Your cost of coverage is based on your age on January 1 of the plan year, your salary at the time of enrollment and the level of coverage you select. The AD&D benefit is a matching benefit to the supplemental life, not a separate election.AD&D rate of $0.0095 per $1,000 included in employee rates. If your spouse also works for the district, you may each have employee supplemental life and AD&D, or one of you may have employee supplemental life and AD&D and the other have spouse life and AD&D, but not both.
For spouse life and AD&D, your cost of spouse coverage is based on your spouse’s age on January 1 of the plan year and the level of coverage you select. Spouse life and AD&D coverage cannot exceed the amount of your supplemental coverage. Spouse coverage is not effective until your supplemental coverage is approved. The AD&D benefit is a matching benefit to spouse life, not a separate election.
AD&D rate of $0.0095 per $1,000 included in spouse rates. The benefit is based on your benefit level and your salary, up to the maximum benefit-the lesser of employee supplemental life and AD&D coverage or $100,000.
Your cost for child coverage is the same no matter how many eligible children you insure. Child coverage is not effective until your supplemental coverage is approved. The AD&D benefit is a matching benefit to child life, not a separate election.
